Listed below on this page, you will find links to various forms that the Youtherans might use throughout the year.  As the time comes for Synod youth events and retreats, those registration forms will be posted here.  Please let us know if you have any questions or concerns.

    • Medical Form and Group Norms
      • All youth will be required to have a medical form and group norms form on file for any and all youth events they attend.   You may download this form in Microsoft Word format.  Please turn them in to a youth advisor when complete.

      • Youtheran advisors are always trying to improve on the youth ministry experience here at St. Mark.  We very much welcome and value parent opinons on how the group is doing.  Please take a moment to share your reflections by filling out the following form and drop it by the church office, or email it to us.  Thank you for your help with this.
